Help with the header/footer in microsoft word please?

I am writing a term paper, and I need a bibliography and title page which do not need a header. the header needs to be my last name and the page number (ex.: Smith 1 on the 1st page and Smith 2 on the 2nd page.) how do I do this? (without putting a header on the first and last pages?)
